Joint Commissioning

Brent Local Authority and Brent Clinical Commissioning Group in partnership with parents. Work stream groups Descriptions Thursday 14th September 2017 10am-1pm

Overall goal – Parent/Carers are meaningfully participating in the design of services through the work programme of the Inclusion Strategic Board.

We are looking for three parents to sit on each group listed below .

You will together as equal partners with professionals from the Local Authority and CCG use your knowledge, skills and experiences as parents with a child or young person with SEND to participate in discussions and inform direction in the review or design of services

You will need to feedback to parents ( we will support you to do this ) to ensure that you are representing as best as you can the views of a wider group of parents along side your own.

1) Transition Planning workstream

Lead – Helen Duncan –Turnbull Head of Adult Services –Complex Care, Community Wellbeing.
Dates To be advised monthly
Duration – 2 hours
Location – Brent Civic Centre
Remuneration will be provided for your time – reading any papers, attendance , at meetings and feedback to wider network of parents through the forum.
Support will be provided by Carol Foyle – Parent Participation lead for Brent LA and Brent parent Carer Forum.

An integrated 14-25 pathway to be developed and agreed with key stakeholders who work with SEND, Parents and Young people. The aim is to ensure there is a coherent protocol which supports successful preparation and progression from school to college and into adult life across all agencies and professionals. This will ensure smooth transition and preparation for adulthood into employment and community to activities that will support them to be independent and to lead fulfilling lives.

2) The Local Offer and Short Breaks

Lead –Stephen Gordon- Head of Localities – Children’s Social Care .
Dates 1st meeting to on held Friday 6th October 10-12pm there after monthly
Duration – 2 hours
Location – Brent Civic Centre ,5th Floor room 3

Support will be provided by Carol Foyle – Parent Participation lead for Brent LA and Brent parent Carer Forum.

The aim is to ensure Parents and Young People are fully aware of and fully involved in future development of the SEND Local Offer and publicising the local offer effectively to all groups of parents and young people. In addition, parents are actively accessing it and providing feedback to improve services.

Short breaks – Consultation on the short breaks and the short break statement to ensure that parents are involved in finalising the statement and clear about the offer and what is available to them. They are also involved in the future development of the short break offer so that the short breaks offer provides increased access to a wide range of services and specialist support.

3) EHC Plans and Processes

Lead –Maria Lunt – SEND Manager.
Dates 1st meeting to be held on Thursday 5th October 10-12pm there after monthly
Duration – 2 hours
Location – Brent Civic Centre, 5th Floor room 3

Increase the participation of parents and children and young people in the co-production of EHC Plans so that an increased number of parents are active participants in the development of EHC plans and agreeing outcomes for their CYP. Parents will also be involved in any future development of the EHC Plans and advise on Quality Assurance processes

To ensure that EHCPs are holistic and cover all education, health and social needs and have measurable outcomes. At the same time ensuring that we adhere to the statutory timescale of 20 weeks to issue a final plan. In addition, we wish to increase participation of parents and children and young people in the co-production of EHC Plans so that an increased number of parents are active participants in the development of EHC plans and agreeing outcomes for their children and young people. Parents will also be involved in any future development of the EHC Plans, Services and the development of a Quality Assurance process.

4) Joint Commissioning

Lead Sandra Bingham (Interim Head of Inclusion and Dionne Greenway Senior Commissioning Manager , Brent Clinical Commissioning Group ( CCG)
Dates 1st meeting to be held on Wednesday 27th September 10-12pm there after monthly
Duration – 2 hours
Location – Brent Civic Centre, 4th Floor room 4

The aim of the Joint Commissioning work stream is to have an Integrated Children and Young People therapy services ( SALT, OT,PT) and nursing services for Children and Young People SEND those on SEN Support and with an EHC plan. The service is to be jointly commissioned and in place for children and young people age 0-25 including the most vulnerable groups e.g. LAC/PRU/YOS/Home educated. We wish to ensure that parents are actively involved in the specification of services for children and young people as well as the Implementation of the Joint Commissioning Framework with the local authority and the CCG

Parents will be actively involved in the specification of services for children and young people and the Implementation of the Joint Commissioning Framework and Plan in Brent with the local authority and the CCG.
