As parents and carers ourselves, we know too well how many forms we are continually asked to complete, so we are really sorry to have to ask you to fill out this one too! We have kept it as short as possible and have only asked for information that will help us:

• Plan information days and other events of interest for parents and carers
• Inform Brent Council & NHS where services are needed but do not currently exist
• Identify which areas are in most need of having a service in their locality
• Keep you updated with progress and developments on services, for children and young people (0-25yrs) with special educational needs and disabilities (SEND) and their families
• Invite you to be on specific focus groups
• Apply for a Brent disability access yellow card from the LA and Max discount card from us
